A wholesale merchant in Moonachie, NJ was recently cited for willful violations relating to exits and electrical regulations and fined over $181,000.
The U.S. Postal Service sorting facility in E. Rutherford, NJ is facing more than $251,000 in fines for repeat violations of OSHA standards covering electrical systems and equipment and emergency exits and exit routes.
A small arms manufacturer located in Bloomfield, NY was cited for violations relating to lockout/tagout, machine guarding, noise and confined spaces in a recent inspection. The facility is facing more than $244,000 in penalties as a result of the inspection that occurred earlier in 2017.
An air conditioning company in Virginia was recently cited for having unsecured boards and unmarked roof hole covers that lead to the death of an employee. The company was fined over $225,000 for this incident, in which they did not correctly inspect the weight that working/walking surfaces could hold, correctly mark roof holes and secure roof hole covers.
A metal processing facility in Michigan is facing over $100,000 in fines for failing to properly train employees on machine operation, providing proper personal protective equipment for these machines (PPE), and having the correct machine guarding installed.
A container manufacturing company has had 15 employees suffer amputations within the past five years and has thus received over $500,000 in fines. The company failed to properly train employees on lockout/ tagout procedures leading to improper or no equipment lockout during cleaning procedures that lead to injury exposure and failing to properly install machine guarding.
Another company failed to have proper lockout/tagout procedures in place that resulted in a finger amputation at a sports equipment manufacturer. OSHA found employees at the facility were often exposed to potential amputation injuries during maintenance operations. The company has been fined more than $65,000 for this incident.
